POSITION TITLE:     Clinical Service Provider

SUPERVISOR:        Treatment Supervisor

QUALIFICATIONS:  Bachelor of Arts of Social Work, Psychology, Counseling, or related field required. 


Master’s Degree with Independent license preferred.

Note: Costs for licensure reciprocity can be covered by the Agency

JOB SUMMARY:      Works with Treatment Supervisor (or Program Director) to provide
comprehensive treatment services to those clients in the Outpatient or Residential Programs.


                                    All employees of Transitions, Incorporated are required:

 1) to arrive on time for their scheduled shift;
 2) to contact the Treatment Supervisor (or Program Director) as soon as
        possible if unable to work a scheduled shift;
3) to maintain a professional attitude and atmosphere through strict
        adherence to the Transitions’ Code of Ethics, Non-
        Fraternization Policy, Infection Control Policy, Fire-Safety
        Policy, Drug Free Workplace Standards, Dress Code, etc., as
        outlined in the Personnel Policies and Appendixes; and

It is essential that the Clinical Service Provider:

  • remain alert and attentive throughout their shift.

  • render emergency first aid and CPR when needed and coordinate emergency efforts in case of fire, tornado, etc.

  • visually identify residents during hourly checks of all areas of the building.

  • communicate (both oral and written) with clients, other staff and the public.

  • respond appropriately to emotional and physical crises of clients.

  • remain aware and cognizant of client activity and activities within the house and monitor signs of client sobriety through visual, auditory, and olfactory observations.







    • Interview and observe clients and prepares clinical reports and individualized service plans 

    • Conduct assessments (including dependent diagnosis), individual counseling, group counseling, and case management.

    • Assure documentation is in compliance with contract regulatory and Agency requirements and best clinical practices. 

    • Use evidenced based counseling methods, in order to maximize treatment process and outcomes (group and individual).

    • Use evidence based interviewing techniques and/or instruments in order to develop an accurate diagnostic summary/service plan.

    • Remain current in the general knowledge of psychotropic medications, side effects and current DSM/ICD-10 diagnoses.

    • Recognize when medical referrals are appropriate.

    • Use information in order to provide accurate diagnosis.


    Attend all Staff Meetings and In-Service Sessions, unless excused by Treatment Supervisor or Program Director.

    Develop and maintain a network of community referrals.

    Represent Transitions in a positive and professional manner in all community interactions.

    Assist in other Transitions’ Programs as needed or assigned.

    Communicate with peers from other Transitions’ components regarding common clients.

    Other clinical, residential or administrative duties as needed or assigned.

     Clinical Staff Productivity Standards:

In order to prioritize the many essential tasks performed by our Clinical Staff and to optimize the experience for our Consumers, Transitions has implemented a Clinical Staff Productivity Policy.   The most important tasks for our Clinical Staff involve the face to face clinical services provided to our consumers, defined as “Productivity”.  Therefore, Staff who provide clinical services have standards for the volume of service production expected of them.


The Productivity Level for the Clinical Service Provider Position is 55%.

This means in a 40 hour work week, a Clinical Service Provider must spend at least 22 hours per week on documented clinical services, as set out in Transitions’ Clinical Staff Productivity Policy.
